Polish Schengen Visa: Form signature, phone/purse, and payment

I have an upcoming appointment at the Polish Embassy in Washington, D.C. I have three questions.

(1) Application signature – date and place

Conflicting advice online: some say sign with the date I filled the online form (back in May), others say sign on the day of the appointment at the embassy.

Can I put my current U.S. state as place and date it 2–3 days before the appointment? Any better approach?

(2) Phone and purse

Will I be allowed to bring a small purse (with passport, ID, credit card) and a switched-off phone? I need these items for identification and payment.

(3) On-site payment

The application form didn’t ask for payment. I assume I pay at the embassy on the day of my appointment. The website says: “Visa fee is XXX USD. The embassy accepts credit/debit card, cash, money order, or cashier’s check.” Is that still correct?

Thank you in advance for any firsthand experience, especially from those who have visited a European embassy directly. But other related advice is also greatly appreciated.
